Place a grievance with the State agency regardless of whether or not a grievance was first placed with the hospital.

Access of care regardless of race, creed, color, national origin, religion, disability/handicap, age, sex, sexual preference, ability to pay or sources of payment.

Receive the services of a translator or interpreter as soon as possible.

Informed of names and functions of all physicians and other health care professionals who are providing direct patient care.

Be informed of your rights and responsibilities in advance of furnishing or discontinuing of care.

Participate in the development and implementation of his/her plan of care.

Make informed decisions regarding his/her care.

Be informed of his/her health status, involved in care planning and treatment, and the ability to request / refuse treatment.

Formulate advance directives and to have hospital staff and practitioners who provide care in the hospital comply with these directives.

Have a family member or representative of choice and his/her physician notified promptly of his/her admission to the hospital.

To be treated with consideration, respect and recognition of their individuality, including personal privacy.

Receive care in a safe setting, free from verbal or physical abuse or harassment.

Be free from restraints and / or seclusion of any form used as a means of coercion, discipline, convenience or retaliation by staff.

Receive appropriate pain assessment and management.

Confidentiality of his/her clinical records and the ability to access information contained in his/her clinical records within a reasonable time frame.

Examine and receive an explanation of the patient's hospital bill.

Hospitalizations

Other matters relating to your health

Report unexpected changes in your condition to your doctor

Report whether you clearly comprehend a contemplated course of action and what is expected of you
Compliance with Instructions:
You are responsible for following the treatment plan recommended by the practitioner primarily responsible for your care. This may include the following:

Instructions given by nurses and allied health personnel as they carry out the coordinated plan of care

Implement the responsible practitioner's orders

Enforce the applicable center rules and regulations

Keeping appointments

Notify the responsible practitioner or hospital if you are unable to go to the appointment
Refusal of Treatment:
You are responsible for your actions if you refuse treatment or do not follow the practitioner's instructions.
Hospital Charges:
You are responsible for assuring that the financial obligations of your health care are fulfilled as promptly as possible.
Hospital Rules and Regulations:
You are responsible for following hospital rules and regulations affecting patient care and conduct.
Respect and Considerations:
You are responsible for being considerate of the rights of other patients and hospital personnel and for assisting in:

You are responsible for being respectful of the property of other persons and of the hospital.
Patient Personal Items
Please leave all valuables, including your wallet, purse or handbag, at home. We have no provisions for their safekeeping and will not be responsible for them.
Please be prepared to pay your co-payments and deductibles, however, please do not bring large amounts of cash with you.
Child and Adolescent:
If the patient is a child or adolescent, the parents or legal guardians shall assume all of the above rights and responsibilities on behalf of the patient.
